Please review the following Terms of Service (“Agreement” or “Terms of Use”), which govern your use of LUMIWHITE.COM (the “Site”) and the shopping and other services (the “Services”) offered on the Site. This Agreement is entered into by and between you (the “patient/s”) and LUMIWHITE Teeth LTD (“LUMIWHITE Teeth Whitening”, “we” or “us”). Your use of the Site and/or registration for any Services will constitute your agreement to comply with the terms of this Agreement. If you cannot comply with this Agreement and its requirements, please do not use the Services and exit the Site.

Privacy Policy

You agree that any information you provide will always be accurate, correct and up to date. LUMIWHITE Teeth Whitening has a Privacy Policy which you can access to understand how we collect, use and handle your personal information.

LUMIWHITE Dentist

A LUMIWHITE Teeth Whitening dentist (“LUMIWHITE dentist/s”) is any UK-based, GDC-registered, dentist who has agreed to see any patient who has signed up for LUMIWHITE Teeth Whitening treatment and any other associated products via this website.

Eligibility

LUMIWHITE Teeth Whitening products may be used to treat a variety of cases including:
• Dietary stains.
• Yellow teeth due to ageing.
• Tetracycline stained teeth.
• Yellow/Brown teeth due to fluorosis.
• Discoloured teeth due to calcific metamorphosis.
• Anterior teeth prior to composite bonding or porcelain veneers/crowns.

LUMIWHITE Teeth Whitening products are not suitable for:
• Patients under 18 years of age.
• Patients who are pregnant or breastfeeding.
• Patients who are not dentally fit – i.e. have caries, extensive and/or deficient restorations, periodontal problems or other problems which may cause hypersensitivity during whitening.
• Patients who want to whiten their crowns, veneers, fillings or dentures. For such patients there may be other treatment options. Please consult your dentist for further information.
• Patients who are not willing to carry out the treatment as directed by their LUMIWHITE dentist.

Additional Treatment

Before completing your purchase, you agree that LUMIWHITE Teeth Whitening is not responsible for any dental treatment required pre-whitening.

LUMIWHITE Teeth Whitening treatment is subject to a full dental assessment by a LUMIWHITE dentist. A LUMIWHITE dentist reserves the right to decline providing LUMIWHITE Teeth Whitening to a patient who is not dentally-fit. Being dentally-fit is defined as having a healthy mouth and, more specifically, healthy hard and soft tissues (e.g. teeth, gums and bone structure) that are free from any disease.

The LUMIWHITE dentist may offer additional treatment options, privately, to help a patient become dentally-fit. The charges for any additional treatment options are solely decided by the LUMIWHITE dentist and may vary between LUMIWHITE dentists across different locations. The LUMIWHITE dentist will discuss payment options, including the availability of finance options (if any), for any additional private treatment/s. Any additional private dental treatment may result in a delay in starting LUMIWHITE Teeth Whitening. If a patient already has a general dentist, they may wish to seek treatment with them prior to returning to see their LUMIWHITE dentist for LUMIWHITE Teeth Whitening.

Late Cancellation or Failure To Attend

Late Cancellations and Failure To Attend appointments have a negative impact on the service we and our LUMIWHITE dentists provide. A Late Cancellation is defined as cancelling an appointment with a LUMIWHITE dentist without providing us with 2 working days’ notice – this can be done by sending an email to [email protected] or messaging us via the LIVE CHAT feature on our website. A Failure To Attend appointment is when a patient does not attend their appointment with their LUMIWHITE dentist and does not let us know they will not be able to attend.

A £30 rebooking fee, payable directly to the dental practice the LUMIWHITE dentist is based at, will be requested should a patient wish to reschedule any of their appointments following a Late Cancellation or Failure To Attend.

Late For Appointments

We understand that some patients will have to travel long distances to see their LUMIWHITE dentist and, in some cases, being late for appointments is unavoidable. However, if a patient is more than 10 minutes late, the LUMIWHITE dentist may no longer be able to see them on that day and the patient may need to rebook. If a patient has to rebook (after turning up more than 10 minutes late), a £30 rebooking fee payable directly to the dental practice the LUMIWHITE dentist is based at will be requested.

Rearranging Appointments

If a patient wishes to rearrange any of their appointments with their LUMIWHITE dentist they must notify us at least 2 working days before their scheduled appointment. Patients can notify us by messaging us via the LIVE CHAT feature on our website or by sending an email to [email protected] Once a patient has informed us that they wish to rearrange their appointment, their LUMIWHITE dentist will contact them to reschedule their appointment.

Dentist Location

Whilst we strive to match every patient with a LUMIWHITE Dentist in their preferred location (and within a 20 miles radius of the provided postcode) we understand this may not always be possible. Patients will, therefore, be provided with the option of seeing a LUMIWHITE Dentist in the next available area. In the event a patient is not happy with the distance required to travel to see their LUMIWHITE Dentist, LUMIWHITE Teeth Whitening will honour a full refund.

Availability of Dentists

LUMIWHITE Teeth Whitening is not responsible for the availability of LUMIWHITE dentists. We will make visible all the locations (cities and towns) of our LUMIWHITE dentists via a dropdown menu on our website. The LUMIWHITE dentist will contact you to book your initial consultation and any further appointments can be arranged directly via them. Providing you do not require any additional treatment prior to starting LUMIWHITE Teeth Whitening, your LUMIWHITE dentist will book you in for your next appointment 5-14 working days (subject to availability) after your initial appointment to fit your LUMIWHITE Superseal Whitening Trays.

B1 Shade Guarantee

LUMIWHITE Teeth Whitening is a dentist-led, professional, teeth whitening system. Patients will be informed by their LUMIWHITE dentist whether they are suitable for LUMIWHITE Teeth Whitening. LUMIWHITE Teeth Whitening is proven to whiten teeth safely and effectively. Whilst the majority of patients can expect to reach a B1 shade (the whitest tooth shade for natural teeth), providing they have used LUMIWHITE Teeth Whitening alongside LUMIWHITE Superseal Whitening Trays as recommended by their LUMIWHITE dentist, some patients may require prolonged whitening to achieve a more desired result.

Subject to availability, every patient will be given the chance to attend a review appointment (between 4 and 8 weeks) after receiving their professional teeth whitening kit from their LUMIWHITE dentist. If a patient does not achieve a B1 shade after the use of 15ml LUMIWHITE Teeth Whitening gel, LUMIWHITE Teeth Whitening will provide the patient with an additional 6ml of LUMIWHITE Teeth Whitening gel for no additional charge – as long as the patient consents to having pre and post-whitening clinical photographs with their LUMIWHITE dentist and provides consent for their LUMIWHITE dentist to share these with us. We will also require a patients pre and post-whitening tooth shade from the LUMIWHITE dentist before honouring any guarantee.

If a patient fails to attend a review appointment with their LUMIWHITE dentist 4 to 8 weeks after receiving their professional teeth whitening kit from their LUMIWHITE dentist, the B1 shade guarantee will not be honoured. LUMIWHITE Teeth Whitening does not take any responsibility for any change in tooth shade beyond a patients final review appointment. If a patient wants to maintain their bright, new, smile they should discuss an appropriate top-up regimen with their LUMIWHITE dentist at their review appointment.

Refunds and Returns

Patients must notify us of any refund requests by sending an email to [email protected]

Should a patient decide they no longer want to proceed with LUMIWHITE Teeth Whitening treatment, they must inform us within 14 days after purchasing LUMIWHITE Teeth Whitening from this site to be eligible for a full refund (as long as they have not yet attended their initial appointment with their LUMIWHITE dentist and provide us with at least 2 working days notice prior to their initial appointment).

Patients will only be entitled to a part-refund (£60 from their order total will be retained by LUMIWHITE Teeth LTD) if they decide they no longer want to have LUMIWHITE Teeth Whitening treatment and the cancellation request was received less than 2 working days before their initial appointment with their LUMIWHITE dentist.

Patients will also receive a part-refund (£60 from their order total will be retained by LUMIWHITE Teeth LTD) if they decide they no longer want to proceed with their LUMIWHITE Teeth Whitening treatment at their initial appointment with their LUMIWHITE dentist.

If a patient fails to attend their initial appointment with their LUMIWHITE Dentist, and decide they want to cancel their order and not rearrange their initial appointment, they will only be entitled to a part-refund (£60 from their order total will be retained by LUMIWHITE Teeth LTD).

No refunds will be provided once dental impressions are taken by the LUMIWHITE dentist to create LUMIWHITE Superseal Whitening Trays for the patient.

Use of Photos and/or Videos

LUMIWHITE dentists may request to take clinical photographs and/or videos of a patients face and/or teeth for clinical, marketing and educational purposes and they may share these with us. A patients name will never be published and identity will never be disclosed without prior permission. If a patient DOES NOT give permission to LUMIWHITE Teeth Whitening and the LUMIWHITE dentist to use their images, please let us know by sending an email to [email protected]

Medical History

It is very important that patients provide their LUMIWHITE dentist with a full medical history and details of any medication they will be taking, are currently taking and/or have taken previously.

Please be aware that any information provided through any part of this website is provided for interest purposes only and does not constitute professional dental/medical advice. For professional dental/medical advice we strongly recommend that you consult your LUMIWHITE dentist.

By using this Site, you are agreeing not to treat any information obtained as being dental or medical advice. You are also agreeing not to use any information on this website to treat, remedy, self-treat or incorporate into other treatments for any dental or medical issues, concerns or problems you or another person may have.
